Most scouts will need to complete the Class 1 & 2 medical form. Fill out the Class 1 part on the front side and the top of the back side. Your doctor should fill out the Class 2 part on the bottom of the back side.
The Class 1 portion is valid for 12 months. The Class 2 portion is valid for 36 months. The appropriate form must be valid at the time of participation.
The Class 3 medical form must be completed by adults age 40 and older, all scouts and adults going to Sea Base and all scouts participating in the K Games program at camp Raven Knob. A portion of the form must be completed by your doctor.
The Class 3 form is valid for 12 months and must be valid at the time of participation. Exception: Any adult staying at Raven Knob for less than 72 hours is only required to provide a Class 1 form.
This form is used to sign-up and submit a $50 deposit for the July 2003 trip to Philmont Scout Ranch in Cimarron, New Mexico.
